Required Forms for Access to Fred Hutch

As an organization with a wide variety of institutional relationships and collaborations, there are many individuals that may require access to our campus or our network that are not on our payroll (non-employees). It's our obligation to maintain strict standards regarding access to our confidential information and study data. To become a non-employee at Fred Hutch, you must have a Fred Hutch employee sponsor and complete our non-employee packet.

  1. Complete the new hire paperwork below. To properly view/complete paperwork, you must have Adobe Acrobat installed [Download free Adobe Acrobat Reader
    Be sure you are ready to print before you start filling out the forms. If you need an alternate method, contact HR at 206.667.4700.
     
  2. Print and sign the completed paperwork.

  3. Bring the completed and signed paperwork to Human Resources as soon as possible OR return these documents to your sponsor who will submit the entire packet to HR.

Non-Employee Information Form

This form provides us with the necessary information to enter a non-employee into our database. Fill out the form completely, initial and sign at the bottom. Social Security numbers and dates of birth will be used only as unique identifiers in our system; they are never released externally. Primary employer and supervisor indicate the company or organization that pays the individual, not Fred Hutch. If there is no outside employer, leave that space blank. Enter the appropriate information and print out for signature in ink.


Agreement Relating to Inventions

This agreement relates to any possibly patentable device, process, product, or discovery (invention) that a non-employee may conceive of during their relationship with Fred Hutch, or in use of Fred Hutch facilities.

Review the entire agreement, then type your name at the bottom to print off for signature in ink.


Agreements and Understanding Relating to Copyrights

This form is an agreement relating to Copyrights that a non-employee brings with them to Fred Hutch and any that may be created during their relationship with Fred Hutch, or in use of Fred Hutch facilities.

A brief summary: the non-employee agrees to promptly disclose to Fred Hutch all writings and other works (such as computer programs) subject to copyright protection that are potentially marketable or saleable (hereinafter Copyright Works) created by them, solely or jointly with others, during the term of their relationship with Fred Hutch.

The non-employee understands and agrees that all copyrights in Copyright Works, partially or fully created by them as part of their relationship with Fred Hutch, or in use of Fred Hutch facilities, shall be, and remain, the property of Fred Hutch.

Confidentiality Agreement

Review this agreement, enter your name and the date, and print off for signature in ink. If you don't have supervisor or division information right now, we can provide that for you during your visit to HR.


Background Check

Fred Hutch is required to obtain information from prospective non-employees regarding healthcare-related offenses. Enter the appropriate information and print out for signature in ink.
