This version of the short-form consent only applies for studies that were approved by the IRB on or after January 21, 2019. If your study was approved by the IRB before January 21, 2019, use the Pre-2018 Requirements version of the short-form. >
Short-form consents are for use when subjects cannot read English and the study did not expect to enroll participants that speak their language. Fred Hutch has short-form consents to participate in a research study available in 50 languages, from Albanian to Vietnamese.
