NHS Funding Modification

This form is only to be used to submit funding modifications on research that has an existing Not Human Subjects (NHS) determination from the Institutional Review Office. Contact IRO with questions.

The only changes allowed to an NHS file are to add funding that does not alter the scope of work in any way. For any other changes, including addition of new specimens or data, you must submit a new, separate NHS request to cover the entire project.
